Physical Information (This information is current for 2010)
A student can have his or her own physician conduct their physical. But, you must take our physical form with you and have it completed by your own physician. The form can be picked up in the Activities Office (preferable) or downloaded from our website. If this form is not dated, stamped and signed by the doctors that you are cleared for participation, it will not be acceptable. Physicals must be performed by an M.D........ Chiropractors, Dentists, and Physician Assistants are not acceptable. Failure to appropriately complete a physical will prevent you from participating in athletics at Edison High School. Any questions concerning physicals should be directed to our trainer Lance von Stade at (714) 962-1356 ext. 4290 .
Summer Camp Emergency Card All new athletes and incoming Freshman must have a completed Summer Emergency Clearance Card. These Cards will be available at your feeder schools, here at Edison Activities Office, the Receptionist desk, or you may download a form HERE. Please bring this completed form with you on your first day attending your summer camp.
Band Camp
